What Is a Partial Hospitalization Program?

The same therapists, the same psychiatrist, and the same case manager, from day one to discharge.

One team Three roles Zero handoffs

Radix Recovery’s Partial Hospitalization Program (PHP) in Cedar Rapids, Iowa offers 4 to 6 hours of structured clinical treatment each weekday while you live at home or in sober housing. PHP is ASAM Level 2.5, the most intensive outpatient level of care. Our PHP Radix community members work with the same clinical team they had in residential treatment: the same therapists, the same psychiatrist, and the same case manager, from day one to discharge.
This clinical continuity is not standard in Iowa, and it matters. When you step down from residential into PHP at Radix, there is no new assessment, no new intake paperwork, and no period where your provider is still learning your history. Your therapist already knows what you are working on. Your psychiatrist already knows your medication protocol. Your case manager already has your discharge plan in progress.
PHP is the right level of care for people stepping down from inpatient residential treatment, or for those whose clinical acuity warrants more structure than standard outpatient care can provide. If you are unsure where you fit on the continuum, our admissions team can assess your level of need over the phone.

PHP is also known as day treatment or day treatment rehab. It provides the structure of inpatient care with the independence of returning home or to sober housing each evening. At Radix, PHP programming runs Monday through Friday, 8:30 AM to 3:30 PM.

Who Is PHP the Right Level of Care For?

PHP is the right choice when you need more structure than standard outpatient can provide, but you are stable enough to return home or to sober housing each evening. Our admissions team typically recommends PHP when:

You have just completed inpatient or residential treatment and need a structured step-down

01
The transition from 24/7 care to independence is where many people lose momentum. PHP bridges that gap with daily clinical structure.

Your withdrawal risk is low, but your cravings and triggers remain significant

02
Medically stable does not mean clinically ready for minimal oversight. PHP keeps the clinical contact high while your body heals.

You are medically stable but have a co-occurring mental health condition that needs daily monitoring

03
Depression, anxiety, PTSD, and bipolar disorder require ongoing clinical attention that standard outpatient cannot provide at the frequency PHP delivers.

You have a supportive home environment, or access to sober living, where you can sleep safely

04
PHP works because you return to a stable environment each night. If housing is a concern, our team can connect you with sober living options near our facility.

Your schedule allows for 4 to 6 hours of daily programming on weekdays

05
Programming runs 8:30 AM to 3:30 PM, Monday through Friday. Evening and weekend employment is feasible for many Radix community members.

A previous IOP or outpatient program was not sufficient to maintain your recovery

06
If you have tried lower levels of care and they were not enough, PHP provides the intensity and accountability your recovery may require.

2.5

ASAM Level

What It Means for You

ASAM Level 2.5 is the clinical designation for Partial Hospitalization Services. It represents the highest intensity of outpatient care, sitting one step below residential treatment (Level 3) on the continuum. Your placement at this level is determined by our admissions team using the full ASAM Criteria, a six-dimensional clinical assessment that considers your medical status, mental health, support system, and readiness for change.

What Does a Day in Radix PHP Look Like?

What Conditions Do We Treat in PHP?

Our PHP is designed for adults navigating addiction alongside the mental health conditions that often travel with it. We treat the full picture, not just the substance.

Substance Use Conditions

Co-Occurring (Dual Diagnosis)

Co-occurring
0 %+

Dual Diagnosis Is the Norm, Not the Exception

More than 50% of people seeking addiction treatment have a co-occurring mental health condition (SAMHSA NSDUH, 2023). Our PHP treats both conditions on one integrated treatment plan, managed by one clinical team. We do not refer out for mental health. We treat it alongside addiction, because separating the two rarely works.

Our Clinical Approach in PHP

Every modality we use in PHP is grounded in peer-reviewed evidence. We do not chase trends. We use what works, and we measure progress weekly.
Core group and individual sessions targeting thought patterns that drive use
Distress tolerance, emotional regulation, interpersonal effectiveness
Collaborative approach to resolving ambivalence about change
EMDR and trauma processing integrated for PTSD co-occurring with SUD
Primary therapeutic modality, 2-3 groups per day in a cohort of peers
Structured sessions with loved ones, available weekly in PHP

Discharge planning, housing, employment, and community referrals

The Same Clinical Team Follows You

Your therapist knows your story. Your psychiatrist knows your medication history. Your case manager knows your discharge plan.

Already knows what you are working on

The relationship you built in residential continues. No new intake. No re-telling your story to a stranger. Your therapist picks up exactly where you left off.

Already knows your medication protocol

The medication plan your psychiatrist established stays in place. No new prescriber learning your history from a chart. Continuity where it matters most.

Already has your discharge plan in progress

The discharge plan your case manager started writing in week one of residential becomes the roadmap for PHP. Housing, employment, aftercare: already underway.

Most treatment programs hand you off at discharge. You get a new intake worker, a new therapist, and a new treatment plan. You spend the first two weeks of PHP just getting everyone caught up. At Radix, that does not happen.

When you leave residential and step down into our PHP, you are not starting over. The relationship you built with your therapist continues. The medication protocol your psychiatrist established stays in place. The discharge plan your case manager started writing in week one of residential becomes the roadmap for PHP.

This is not just a scheduling convenience. Clinical research shows that therapeutic alliance, the relationship between a person in recovery and their treatment provider, is one of the strongest predictors of long-term outcomes. We protect that relationship at every step.
